Topics Discussed in the Lecture:

1. First Trimester Fetal Evaluation: Key Concepts and Indicators

  • Daily growth timeline of 1 mm for Crown-Rump Length (CRL)
  • Understanding and monitoring increased Nuchal Translucency (NT) at 10 weeks
  • The Nasal Bone to Prenasal Thickness (NB:PNT) Ratio as a marker for Down Syndrome
  • Intracranial translucency and the double line sign as indicators of mid-sagittal fetal brain development
  • Interpreting tricuspid regurgitation and ductus venosus waveforms
  • Frontomaxillary angle, inferior facial angle, and other soft craniofacial markers
  • Risk calculation based on markers (NT, NB, DV, TR) using specialized software

 2. First Trimester Biochemical Screening and Integration

  • Double marker test (β-hCG, PAPP-A)
  • Enhanced or quadruple marker tests (including PlGF, AFP)
  • Comparing sequential and combined screening strategies
  • How to assess risk using multiple testing methods

3. Second Trimester Soft Markers of Aneuploidy (SMFA)

  • Nine major markers to note:
  • Unossified nasal bone
  • Ventriculomegaly
  • Thickened nuchal fold
  • Aberrant right subclavian artery (ARSA)
  • Echogenic bowel
  • Pyelectasia
  • Echogenic intracardiac focus
  • Short femur
  • Short humerus
  • Grouping markers by body region and likelihood ratios (6.5 to 1)

4. TIFFA Scan and Comprehensive Fetal Organ Review

CNS evaluation: Examining axial, coronal, and sagittal brain structures

  • Key features: CSP, corpus callosum, vermis, sulci, and fissures
  • Anterior and posterior brain complexes
  • 21 neurosonographic assessment parameters

Face evaluation: Nasal bone, jaw, lips, chin, palates, and craniofacial angles
Cardiac screening:

  • Checking abdominal situs, 4CH view, 3VT, RVOT, LVOT
  • Utilizing 3D/AI-enhanced imaging
  • Circle & sausage view, hammock view, great artery arches

  • Differentiating between SGA and FGR
  • Stages of FGR (1 to 4) with related Doppler changes:
  • Umbilical artery, MCA, aortic isthmus, ductus venosus, uterine artery
  • Timeline of Doppler abnormalities in early vs late FGR
